How to use your Health NZ subsidy funding for purchases with Shine Breastcare
If you wish to use your Health NZ Breast Prosthesis Subsidy Funding, then we advise that the first step you could take, is phoning Health NZ to find out how much funding you currently have available at this time.
Dial 0800 855 066 and follow the prompts #3 and #5. Make sure to write down your balance available and the expiry date of your funding balance. This info will be provided by the Health NZ call centre on the numbers above.
For Reimbursement after Your Online Purchase
- Use your credit card to make your online purchase.
- Shine Breastcare will process your order within 3-5 business days unless there is a back order delay.
- Goods arrive at your nominated address.
- Download and print the MOH Claim Form.
- Fill out the sections highlighted in yellow.
- Send the completed forms to us via email or post, & include your funding details:
- Email – Scan & send the form to hello@shinebreastcare.co.nz
- Post – Mail the form to Shine Breastcare. PO Box 24-341, Royal Oak, Akld 1345
- Make sure to also include; 1) Your purchase order number from Shine, and 2) your bank account details for us to process your reimbursement
- After verifying your details, Shine Breastcare will lodge the claim with MOH. The reimbursement process typically takes about 3-4 weeks.
